What to Expect

Before your exam we will ask you to send us:

• Your birthdate, address, and other information

• a photograph of your photo ID card and health insurance card

  (we do not charge for our services, but if you need vaccinations or lab work you may bill that to insurance)

Please bring to your exam:

• Your Photo ID card

• Any other pertinent medical information you might have, such as out of state/country vaccinations, or history of TB treatment, etc.

After your exam:

• Please get any lab tests or additional vaccinations as ordered by our physicians

• Send proof of vaccination or lab results to our physicians via WhatsApp or Email

• After all vaccines and lab results are finished, we will call you to come pick up your final paperwork.
